Personally, Sam Asante from Vissol is my favorite designer but I he's not taking any projects as far as I know. Your budget is an important factor in all of this because $500 is the bare minimum for a decent design.

I have found prices to be anywhere from $250 (Poor quality) to $5000

Check out http://rhykker.com I've had him design my website (you can see the design over here). This guy also happens to be the graphic artist for GameFront (gaming news website) and he also writes articles for them too. His name is CJ Miozzi. If you do use him, tell him PAL-18 referred you.
